Case Management Services

Within 72 hours of admission, you will meet with your case manager for the first time. After that, you will meet with your case manager twice a week. Your case manager will help you create a “Goal Plan” for your stay at Avenues and your future after you leave the program. The goal plan is created by you—you are the expert on your life and know what you need to do in order to be happy and achieve your goals. Your plan will address the following components—you will be asked to come up with goals around these areas:

  • Immediate Issues (these could be things like contacting family, taking care of police or other legal matters, managing immigration issues)
  • Medical and Emotional / Mental Health Concerns
  • Personal Goals—Are there things you want to work on? Anger Management, Sleeping Patterns, etc.
  • Obtaining Vital Documents
  • Employment
  • Education
  • Housing
  • Community (here at Avenues)
  • Independent Living Skills (ILS)
